Cautions

Total and Direct Bilirubin: Total and direct bilirubin results are falsely decreased in the presence of hemolysis. AST (and to a lesser extent ALP and ALT) may be significantly falsely elevated in the presence of hemolysis.  In some cases of monoclonal gammopathies (especially Waldenstrom's macroglobulinemia with very high concentrations of IgM) total bilirubin, and direct bilirubin results may be unreliable. Total and direct bilirubin results may be falsely low in the presence of high concentrations of N-acetyl-p-benzoquinone imine (metabolite of Acetaminophen).
ALT: Falsely low ALT results may occur in patients treated with sulfasalazine.
ALP: Samples with glycine, monoethanolamine, and high concentrations of phosphate and chloride may interfere with ALP measurements.

ALB

Colorimetric; Bromocresol green.

 

DBIL

Photometric with diazonium salt, 3,5-dichloroaniline (DPD).

 

TBIL

Photometric with 3,5-dichlorophenyldiazonium tetrafluoroborate (DPD), and caffeine and a surfactant as accelerators.

 

ALP

Photometric rate with p-nitro-phenylphosphate (pNPP) in the presence of 2-amino-2-methyl-1-propanol (AMP) at pH 10.4.

 

ALT

Photometric rate with alanine to α-oxoglutarate to form pyruvate and glutamate. This assay does not contain pyridoxyl-5-phosphate.

 

AST

Photometric rate with aspartate and α-oxoglutarate to form pyruvate and glutamate. This assay does not contain pyridoxyl-5-phosphate.

 

TP

Colorimetric with cupric ions in an alkaline solution.
