Clinical Information

Includes:  relative and absolute reticulocyte counts, immature reticulocyte fraction (IRF), and reticulocyte hemoglobin equivalent (RET-He)

Reticulocyte counts are indicators of bone marrow erythropoietic activity.

IRF is also an indicator of erythropoiesis, and increases earlier than the absolute reticulocyte value. Potentially useful in monitoring the efficacy of therapy for nutritional anemias,  after treatment recovery from chemotherapy and bone marrow transplant, and erythropoietic stimulating agents. 

RET-He is a marker of cellular hemoglobin in reticulocytes and the recent functional availability of iron. Unlike ferritin, RET-He levels do not significantly vary during inflammation or infection. Normal to high RET-He levels have shown negative predictive value in ruling out iron deficiency.  A low RET-He value does not differentiate an absolute iron deficiency from a functional iron deficiency (inability to mobilize iron from storage sites).
