Clinical Information

Because the concentration of vancomycin fluctuates considerably depending on the time from last dose, interpretation of the clinical significance of the vancomycin concentration must take into consideration the timing of the blood specimen.

Please draw level as scheduled. For patients on continuous infusion only, use the following instructions: 

If vancomycin infusing through central line: ensure vancomycin infusion is stopped for 30 seconds, flush line, and then obtain sample if drawing sample from the same central line as the vancomycin is infusing.

If vancomycin infusing through peripheral line: draw sample from peripheral site opposite or distal from peripheral line through which vancomycin is infusing.

Method Description

Serum or plasma is mixed with Reagent 1, which contains vancomycin labeled with the enzyme glucose-6-phosphate dehydrogenase (G6PDH). Subsequently, Reagent 2, which contains antibodies to vancomycin and the coenzyme nicotinamide adenine dinucleotide (NAD), is added. Vancomycin in the sample and vancomycin-labeled G6PDH compete for antibody binding sites. Enzyme activity decreases upon binding to the antibody, so the vancomycin concentration in the sample can be measured in terms of enzyme activity. Active enzyme converts oxidized NAD to NADH, resulting in an absorbance change that is measured spectrophotometrically.
