Test Code LAB119 Protein Electrophoresis Serum, with Reflex
Clinical Information
Serum Protein Electrophoresis (SPE) is used for screening patients with suspected monoclonal gammopathies, but it alone is not considered an adequate screen for monoclonal gammopathies. For suspected monoclonal gammopathies, serum free light chains are recommended to be ordered together with SPE. Fibrinogen will migrate as a distinct band in the beta-gamma fraction, so plasma samples are not acceptable. Radiographic agents may produce an uninterpretable pattern. Immunotyping of monoclonal (M-) proteins identifies the monoclonal immunoglobulin heavy chain type (gamma, alpha, mu, delta, or epsilon) and light chain type (kappa or lambda) in serum. If a monoclonal light-chain is detected in the absence of an associated monoclonal heavy-chain, immunofixation electrophoresis specific for delta and epsilon chains is performed. Immunofixation is not a quantitative assay. If a monoclonal protein is identified, a serum protein electrophoresis assay is required for quantifying the abnormality.
