Test Code LABFENTQ Fentanyl, Urine, Confirmation

Clinical Information

This test is intended to confirm the presence of fentanyl and its metabolite, norfentanyl, in urine. Detection of these drugs in urine indicates recent exposure to fentanyl but cannot be used to determine the dosage or route of administration.

Synonym

  • FENTQ

Specimen Required

Preferred Container/Tube: Urine Container
Acceptable Container/Tube: Urine Container
Specimen Volume: 5 mL
Specimen Minimum Volume: 1 mL

Specimen Stability Information

Ambient: 21 days
Refrigerated: 28 days
Frozen: 28 days

Rejected Due To

  • Quantity not sufficient

Reference Values

None Detected

Cutoff concentrations by liquid chromatography-tandem mass spectrometry:

Fentanyl: 2.5 ng/mL
Norfentanyl: 2.5 ng/mL

Interpretation

Numeric results (positive) indicate the measured concentration of drug in the urine sample. Negative results may reflect the absence of drug or drug present below the assay's limit of quantitation. The detection of drugs in urine is dependent on several factors including the amount of drug ingested, time since exposure, individual metabolism, kidney and liver function, urine concentration, and the assay's limit of detection.  Drug concentrations in urine cannot determine the dose, route of administration, or exact time of exposure.

Cautions

Detection of drugs in urine is dependent on urine concentration. False negative results may occur in urine that has been diluted either by direct sample tampering or by ingesting large quantities of water before sample collection.

Method Description

Liquid Chromatography Tandem Mass Spectrometry (LC-MS/MS)

Test Classification

This Liquid Chromatography/Mass Spectrometry (LC/MS/MS) test was developed and its performance characteristics determined by the Toxicology Laboratory at The Ohio State University Wexner Medical Center. It has not been cleared or approved by the FDA. The laboratory is regulated under CLIA as qualified to perform high complexity testing. This test is used for clinical purposes and should not be regarded as investigational or for research.
